Behavioral Health Consultant
The Wahpikisik Integrated Health (WINH) team at Rocky Boy Health Center is dedicated to the emotional, psychological, and spiritual health of our community through services delivered with respect and compassion. Behavioral Health Consultants (BHCs) work as a member of an integrated treatment team and alongside other providers and departments to provide holistic, evidenced-based care. The BHC with prescribing abilities serves a key role as a provider and a consultant on teams within the WINH department to, together, help patients achieve their individualized treatment goals. The BHC prescriber would work within the Integrated Behavioral Health department providing evidence-based, interdisciplinary care to address chronic conditions.
KEY RESPONSIBILITIES:
- Provide expertise on psychiatric presentations, psychotropic medications, comorbid conditions, and potential medication interactions. Develop integrated care plans with primary care team.
- Serve patients at the Rocky Boy Health Center (RBHC) through warm-handoffs, co-consultations, assessments, and follow-up/monitoring as a member of the patient’s care team.
- Carry a caseload of patients with Opioid Use Disorder (OUDs) and work effectively with the Integrated Behavioral Health team to support their needs through Medication Assisted Treatment (MAT)
- Participate in team huddles to coordinate care, provide education and guidance to team members, and assist with developing creative solutions to address patients’ complex needs.
- Provide education and training to RBHC staff and the community on subjects such as behavioral health, wellness, and the role and benefits of Integrated Behavioral Health in the Primary Care setting.
- Conduct psychiatric diagnostic assessments with children and adults and recommend medications when appropriate. Conduct regular follow-up assessment of therapeutic side effects and effectiveness and consult with the patient’s Primary Care Provider (PCP) for dose adjustments as needed.
- Perform risk assessment and crisis management, including evaluation and treatment and/or referral for patients who are suicidal, homicidal, gravely disabled due to a psychiatric disorder, or experiencing intimate partner violence.
- Provide support, education, and counseling to family members of clients as clinically appropriate to help them become more knowledgeable about mental illness, collaborate in the treatment process, and assist in their family member’s progress.
- Provide patients with self-management skills and tools to enhance their participation in their own treatment and recovery.
- Assist patients with tasks associated with general functioning; help patients access resources for basic needs and community supports.
- Engage in regular, bidirectional communication with the team and medical providers regarding care plans, treatment recommendations, and progress within a shared care plan.
- Attend collaborative team meetings, promote a trauma-informed approach and cultural sensitivity in managing behavioral components of health conditions.
- Support high-utilizing patients via collaborative care meetings and work with the patient and family to identify behavioral changes that could help them address their chronic health conditions (e.g. SMI, co-occurring disorders, Type 2 Diabetes, hypertension).
- Seek consultation as needed and communicate with outpatient psychiatrists regarding patient needs, complexities, and/or transfer of care.
- Stay up-to-date on evidence-based practices for improved health in integrated care settings.
- Document all patient interactions in the Electronic Health Record according to RBHC policies and timelines.
- Maintain confidentiality according to HIPAA requirements for all patient communications.
- Comply with all requirements as a mandated reporter.
- Make recommendations on outcomes and program evaluation. Assist with related data collection.
- Assist with related projects and initiatives including program development and Quality Improvement.
- Maintains credentials and licensure with all major insurance companies and licensing boards with the assistance of RBHC credentialing and billing staff.
- Other duties as assigned.
MINIMUM QUALIFICATIONS:
- Master’s degree in mental health counseling or social work, nursing with advanced practice certification in Psychiatric care.
- Current valid Montana licensure to practice in the above fields.
- Thorough professional knowledge of the practice of behavioral health.
- No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid)
- Strong interpersonal skills and ability to effectively work as a member of an interdisciplinary team.
- Compliance with all state and federal laws and regulations as they pertain to position including HIPAA, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc., as well as by standards of professional ethics.
- Ability to understand and promote the mission, vision, and values of RBHC both in the work place and in the community.
- Understand a trauma-informed approach to healthcare and the importance of culturally-sensitive practice
- Strong organizational and time management skills.
- Computer literacy including word processing and e- mail.
