Position Summary
The Wahpikisik Integrated Health (WINH) team at Rocky Boy Health Center is dedicated to the emotional, psychological, and spiritual health of our community through services delivered with respect and compassion. The Integrated Behavioral Health (IBH) team works alongside the clinical team to provide evidence-based, interdisciplinary care with a focus on models that effectively address chronic diseases, including Opioid Use Disorder (OUD). The IBH Nurse Case Manager works within the Integrated Behavioral Health department providing evidence-based, interdisciplinary care to address chronic conditions. This positions works with patients both in clinic and in the Chippewa Cree Justice Center.

Major Duties
  • Contribute to the wellness and effectiveness of the Wahpikisik Integrated Health Division through positive communication, ownership, and offering one’s support, strengths and perspectives in a respectful and meaningful way.
  • Work along side the IBH team members to provide direct medical care, both in the clinic and in the community.
  • Conducts assessments with patients and assists in triaging patients requesting services in the jail.
  • Develops, monior, and update patient care plans and communicate progress with the patient and treatment team.
  • Work closely with patients, other providers, and other entities to ensure patients received appropriate and timely care.
  • Foster a collaborative relationship with patients through care plans and compassionate, comprehensive, individualized care.
  • Facilitiate timely, open communication between patients, families, providers, and other entities.
  • Serves as liasion between the clinic and other tribal, regional, and state entitites, including the court systems and other healthcare facilities.
  • Provide education to patients and their families/supports about their health conditions, treatments, and management strategies.
  • Responsible for medication reconciliation for the IBH team including prior authorizations and communicating regularly with the pharmacy.
  • Assists with data collection, including data for grant purposes and data related to patient outcomes, so that decisions are data driven.
  • Communicate with the IBH Team Lead about gaps and needs in the services provided so as to improve the function and effectiveness of the team.
  • Adheres to all local, state, and federal laws, rules and regulations, as well as RBHC policies and relevant ethical standards. This includes maintaining patient privacy and safeguarding protected health information and compliance with the Health Insurance Portability & Accountability ACT (HIPAA).
  • Collaborate with Tribal, State, and Federal Courts as well as other agencies and entities to provide comprehensive and coordinated care.
  • Will serve as a mandated reporter for suspected child or elder abuse and/or neglect and work with supervisor to respond according to mandates.
  • Undertake additional responsibilities as delegated to support the mission and goals of the Rocky Boy Health Center.
Minimum Qualifications
  • Bachelor's Degree in Nursing (BSN)
  • Licensed as a Registered Nurse in the State of Montana
  • Prior experience in medical or other healthcare settings
  • Understanding of chronic disease processes
  • Ability to work independently and as part of a team
  • Must possess a current drivers license
Desired Qualifications
  • Master's Degree in Nursing (MSN)
  • Certification in Case Management (CCM or ACM)
  • Two (2) years of related experience in nurse care management and/or integrated behavioral health
  • Strong organizational and time-management skills
  • Understanding of and appreciation for the tribal customs, traditions, practices and the sovereign status of the Chippewa Cree Tribe
