Care Coordinator
Position Summary
The Care Coordinator provides outreach, care coordination, health education, and disease prevention services to patients in the Rocky Boy and Box Elder communities. This position works closely with patients, healthcare providers, and community resources to help patients access services, follow care plans, and address barriers to their health.
Key Responsibilities
- Coordinate patient care, referrals, transportation, and follow-up services.
- Conduct telephone and in-person outreach to patients.
- Assist patients in developing and following individualized care plans.
- Provide education on preventive care, screenings, medications, and self-management.
- Coordinate pre-admission and post-discharge follow-up.
- Connect patients with community resources, including social services and housing assistance.
- Maintain accurate documentation and required patient outreach records.
- Work collaboratively with providers, nursing staff, and other RBHC programs.
Minimum Qualifications
- High school diploma or equivalent.
- Current CPR and First Aid certification.
- Ability to read, write, and communicate effectively in English.
- Knowledge of the Rocky Boy Reservation, its communities, and local health needs.
- Ability to work independently and collaboratively with healthcare professionals.
- Current valid Montana driver’s license.
Desired Qualifications
- Knowledge of Chippewa Cree culture and traditions.
- Strong communication, organization, and interpersonal skills.
- Experience working with patients, healthcare programs, community outreach, or care coordination.
- Ability to work effectively with individuals from diverse backgrounds and help patients navigate healthcare and community services.
