Peer Support Specialist
The Peer Support Specialist is a member of the Wahpikisik Integrated Health (WINH) team, supporting patients through a biopsychosocial and spiritually informed model of care. As part of the Tribal Assertive Community Treatment (TACT) program, this role provides community-based, team-driven support to individuals with behavioral health needs, promoting recovery, self-efficacy, and engagement through lived experience and peer-based services.
Key Responsibilities
- Provide one-on-one and family peer support to individuals in recovery
- Assist with intake, treatment planning, and ongoing care coordination from a peer perspective
- Deliver community-based services, including in-home and in vivo support
- Support patients with practical needs such as transportation and access to resources
- Participate in interdisciplinary team meetings and contribute to treatment planning
- Conduct outreach and engagement for individuals with serious mental illness (SMI)
- Facilitate transitions to higher levels of care when needed
- Perform documentation and patient tracking in the EHR within required timelines
- Serve in on-call rotation for after-hours TACT support
- Educate patients and community members on mental health, recovery, and reducing stigma
- Maintain compliance with HIPAA and all applicable regulations
- Licensed or license-eligible as a Peer Support Specialist in the State of Montana
- Demonstrated cultural awareness and sensitivity
- Interest in working within an integrated, team-based care model
- Valid Montana driver’s license
- Licensed Peer Support Specialist with at least one (1) year of experience
- Experience working with individuals with mental health and/or substance use disorders
- Experience working with individuals with Serious Mental Illness (SMI)
- Experience in healthcare or integrated care settings
- Familiarity with Electronic Health Records (EHR) systems
- Understanding of and respect for tribal culture, traditions, and community values
